2009 Lakefair Grand Parade Entry Application

Welcome All Parade Applicants!                

As your Parade Committee Chairs, we are honored you are taking the time to complete this on-line application to become a part of the 2009 Capital Lakefair Grand Parade.  Our theme this year is "Teddy Bears' Delight".

In completing this application, here are a few items of particular importance: 

FIRST, please fill out the application forms COMPLETELY.  If you have ANY questions, please call the Capital Lakefair office. 

SECOND, please note that insurance and release requirements are MANDATORY.  Copies of insurance certificates or other proof of insurance and your signed release must be received by the Capital Lakefair office by June 30, 2009 NO ACTION WILL BE TAKEN ON YOUR PARADE ENTRY APPLICATION UNTIL ALL REQUIREMENTS ARE MET AND FEE PAID (IF APPLICABLE).

THIRD, please submit your 100 word or less unit entry description with this affidavit.  This dialog MUST accompany your application.  It is used for television and parade route announcer script books which must be completed well in advance of the parade.  Capital Lakefair reserves the right to edit and modify all dialog to meet production requirements.

FOURTH, Submittal of this application DOES NOT automatically guarantee entry in the Capital Lakefair Grand Parade.  You will recieve notification by no later than July 8, 2009 of your acceptance or non-acceptance into the parade.  If your entry is accepted, you will be sent by e-mail, Final Instructions and Maps.  It is imperative that you provide an accurate and active email address where requested in the application below.  If we do not have this email address NO FURTHER ATTEMPT will be made to contact you

We encourage EARLY applications.  Capital Lakefair reserves the right to prioritize entrants based on the date of receipt of their application if the number of applications exceeds parade capacity.  Capital Lakefair also reserves the right to select parade entrants based upon applications submitted without resort to extraneous information.  Prior parade participation does not guarantee automatic acceptance.  The decision of the Parade Committee in accepting or rejecting applications is final.

Commercial  - A $125.00 Entry Fee is required for Commercial entries.  No action will be taken

                             on your application until fee is received.  This fee must be received No Later

                             than June 30, 2009.   Failure to submit fee will disqualify your application.  

                            If for any reason your application and your entry into the Lakefair Grand Parade

                            is not accepted this fee will be refunded in its entirety.

    your entry can not be described along the parade route or on TV. PLEASE INSURE the dialog is incluced

    with this submittal.  DO NOT SEND AS A SEPARATE SUBMITTAL as it may not be associated with this

    submittal.  If you are not sure of your parade dialog, DO NOT SUBMIT THIS APPLICATION until you have

    your dialog completed.

1.  Please ensure that all pertinent information concerning your parade entry is entered and correct prior to submitting this application.  This includes your Parade Dialog.

3.  If you have not received a receipt of your Parade Application after 7 work days of transmittal, please send an email to parade@lakefair.org  for confirmation.
