Some Lakefair Activities / Events may require an application or registration to enter.
In addition to an application or registration one or both of the following forms may be required:
Evidence of Insurance,
Release of Liability Form
It is your responsibility to provide these forms if required. Failure to provide required form(s) may delay processing of your application/registration or may disqualify your application/registration completely.
